Our First Conversation

We often recommend that, regardless of situation, contacting us first is generally a best initial step as we can guide you through each and every step. You can expect the utmost respect and care in our first conversation where we aim to gather some core information such as: Name of Deceased, Location of Deceased, Date & Time of Death [If Known], Personal Physician’s Name, etc.

Vital Statistics

We are required by law to file the death certificate in the jurisdiction where death occurred prior to services and/or disposition of the deceased. We do rely on you to provide us with accurate information. Please take some time to complete our Vital Statistics form which is by the easiest way to provide the required information. We are, of course, available to walk you through this 24/7.
